Practice Policies

Mental health treatment is one of the most important investments you can make for yourself and your family. With an effort toward optimal treatment success please review following guidelines:

• Karyn Horowitz, MD, LLC is a private pay practice and does not currently accept insurance. Patients are required to pay at the time of service. To simplify this process a credit card may be kept on file.

• A Missed Appointment/Late Cancellation is defined as a cancellation with less than 24 hours notice prior to your scheduled appointment.  

  • If you need to cancel an appointment for any reason, please provide at least 24 hours notice, otherwise your cancellation will be considered “late.”

  • A first late cancellation/missed appointment will be waived as a courtesy.

  • A second late cancellation/no show will be half of your full appointment fee.

  • A third late cancellation will be the full appointment fee. 

  • If you miss more than three appointments without notification, you may be discharged from the practice.  

• Patients will be provided with a super-bill including the billing CPT code(s) appropriate for the completed appointment. These may be submitted to your PPO health insurance company. 

• Please see the "Treatment" section for specifics regarding the cost of services unless a different rate has been negotiated. Fees upon entry are fixed for one year. After that, fees may be raised no more than annually.

• Minimum frequency of appointments will be determined together in our treatment plan.  In order to remain an active patient, it is essential to consistently meet at the expected frequency. The typical frequency of appointments ranges from weekly to every few months depending on your treatment plan and goals.  If there are safety or medical concerns that warrant more frequent visits, this will be communicated.
